Special Attention

1.Applied Documents

a.  The applicant must use the application forms containing information both in Chinese and in English as defined by the Chemicals Registration Center of the State Environmental Protection Administration. Lists prepared in other forms are invalid.

b.  One printed forms and one electronic forms must be submitted one time.

c.  The printed page1and page2(with the original signature) should be submitted only one for one applicant.

d.  The  copies of the Chinese Custom Declaration should be  numbered. The number must be filled in the form.

e.  Every substance which the identification is kept confidential should be have a confidential sheet with the original signature.

f.   The identification of the chemical substance which has already been included in the “National Chemical Inventories” can’t be kept confidential.

g.  If the information needs to be provided with a mark, the mark “X” should be used.

h.  Mixture need not be applied.

i.    Hydrates need not be applied.

j.    The application of polymers should use Form C.

2.The information of the page1 and page2

a.The original signature of the chief responsible person of the applying unit is requested in the column of “Signature” in the first page of FormB or FormC.

b.The mailing address of the native applicants should be provided in Chinese.

c.The address of the technical person to contact specified in page 2 of the application form 

   must therefore be detailed and accurate.

3.The information of the chemical substance

a.The Chinese name is necessary in the form.

b.CAS name or IUPAC name should be used in the Chemical name in English.

c.Trade names or codes are not allowed in the column of “Chemical Names”.

d.The chemical name in Chinese should be named according to “Inorganic Chemicals Named

   Principle” and “Organic Chemicals Named Principle” from China National Chemical

  Association.

e.The CAS number must be correct.

f.The requests of inputting the name of the applied substances refer to the appendix.

g. Item 5.2 “Plant site” that is chosen in the column of “Confidential claim” must be item 4

that has already been filled in. If the reporting chemical substances are produced in China,

the name and address of the plant must be filled in Item 4. If not, item 4 should not be

filled in and items 5.2 “Plant site” should not be chosen.

h.If the identification of the applied substance in Form C needs to be kept confidential, each

   substance should have different generic name of the substance.
